The following forms are available online or in PDF format:
- Housing Questionnaire (For visits >= 3 work weeks)
- Citizenship Form -- As of 4/08 this form is no longer required for U.S. citizens.
- Declaration of Immigration Status -- This form must now be completed by all non-U.S. citizens, including U.S. permanent residents. You may bring it with you to the KITP or fax it to the number below prior to your arrival.
- J-1 request form
- The J-1 application procedure has changed. If you wish to apply for a J-1 visa, please email Timber Kelley directly, and she will tell you how to proceed.
